Join the Society of Clinical Surgery (old)

The Society of Clinical Surgery is now accepting membership applications for 2019/2020.

All applications must be submitted through the SCS Membership Application Submission web form at the bottom of this page, according to the instructions below. Completed applications are due by July 30, 2019, 11:59 p.m. PST. Applications submitted after this date will not be considered.

Please read the following requirements and instructions carefully before submitting your membership application and materials.

New Requirements:

  • It is now required that all membership applications be submitted using the online SCS Membership Application Submission web form, which is included at the bottom of this page.
  • The web form also requires that the following items be completed and saved as a single composite PDF file, which is to then be uploaded and submitted using the online SCS Membership Application Submission web form at the bottom of this page, according to the instructions for “How to Apply” below:
      • Proposal for Membership PDF document, which includes the full SCS membership form (Pages 2-5) and the proposers for membership form (Page 6).
      • Two signed letters of recommendation from SCS members proposing the applicant for membership.
      • The CV of the applicant.
  • There is also a new requirement to include a brief (150 words max) applicant bio, which must also be submitted using the online SCS Membership Application Submission web form at the bottom of this page.

How to Apply:

  1. SAVE your 150-word applicant bio as a Word document or a PDF.
      • Note that the applicant bio should highlight your scope of clinical practice and annual RVUs, educational accomplishments and research contributions specifically focusing on funded projects and those of particularly high impact on the field.
  2. DOWNLOAD the Proposal for Membership PDF document.
      • The Proposal for Membership document is an editable PDF that includes the full SCS membership form (Pages 2-5) and the proposers for membership form (Page 6).
      • Note that the proposers for membership form on Page 6 of the Proposal for Membership document requires signatures from you (the applicant) and your two proposers.
  3. COMPLETE all fields within the Proposal for Membership document and then SAVE the completed document as a new PDF.
  4. COMBINE and SAVE the following items as one, single PDF file: (If you do not know how to do this, see instructions for combining/saving multiple PDFs as one PDF in Adobe Acrobat.)
      • Your completed Proposal for Membership document.
          • You should already have this saved as a PDF after downloading and completing it during Step 3.
      • Your two required letters of recommendation proposing you for membership.
          • Note that each letter of recommendation should come from an SCS member and should comment on your national/international recognition within your specialty, your clinical performance, your innovation and your contributions to your institution and your field.
      • Your CV.
  5. UPLOAD the composite PDF that you created during Step 4 using the file uploader in the “Application Document” field of the SCS Membership Application Submission web form at the bottom of this page.
  6. UPLOAD your 150-word applicant bio using the file uploader in the “Applicant Bio” field of the SCS Membership Application Submission web form at the bottom of this page.
      • This is the file that you should have already saved as either a Word document or PDF during Step 1.
  7. COMPLETE all remaining required fields included in the SCS Membership Application Submission web form at the bottom of this page.
  8. CLICK the “Submit” button at the end of the SCS Membership Application Submission web form at the bottom of this page.

The deadline for applications is July 30, 2019, 11:59 p.m. PST. Upon completing and submitting the SCS Membership Application Submission web form at the bottom of this page, you will receive a confirmation email, and your application and materials will be sent to:
